Browlifts for Migraine Relief

It sounds like a hoax, but I can assure you, it's not. Cleveland plastic surgeon, Dr. Bahman Guyuron, thinks he may have found a revolutionary way to end migraines: the browlift. That's right -- the same surgery used to remove signs of aging from the upper part of the face.

Nearly 20% of women suffer from migraines at some point in their lives. A National Library of Medicine study reveals that they cost employers about 13 billion dollars a year -- quite a high price tag, to say nothing of the patient's pain and suffering. And until now, there has been no cure.

Dr. Guyuron says that his discovery came as the result of a conversation with one patient in 1999. During a routine checkup, this woman, still recovering from a browlift, told him that since undergoing the surgery, her migraines had stopped almost entirely.

Dr. Guyuron decided to look into the matter, and he discovered that most of his patients with migraines had similar stories. He later told CNN that his patients have said, again and again, "Not only am I happy with what I see, but I haven't had headaches in a while."

So, what's the connection? Well, as you may know, browlifts are designed to revitalize drooping or wrinkled foreheads, which can make a person look angry, sad, or tired, as he or she ages. During this surgery, the muscles above the eyes are lifted to smooth lines and raise eyebrows, resulting in a more youthful appearance. In medical terms, this is a resection of the corrugator supercilii muscle. Some doctors now think that this muscle may put pressure on nearby nerves that can, in turn, cause migraines.

Other physicians have doubts. Dr. Seymour Solomon, a professor at the Saul R. Korey Department of Neurology at Montefiore Medical Center, commented on CNN that the surgery is "completely contrary to what we know about the mechanism of migraines."

Other doctors theorize that the surgery may somehow affect the trigeminal nerve, which is known to play a role in the migraine response. The official Mayo Clinic description includes the statement that "during a migraine headache, serotonin levels drop. Researchers believe this causes the trigeminal nerve to release substances called neuropeptides." Thsee neuropeptides cause inflammation of blood vessels in the brain, and your migraine has begun.

Some researchers feel the chemical imbalance (serotonin drop) leads to the nerve response. Others think that perhaps pressure on the nerve causes the chemical changes.

Dr. Guyuron thinks that his observation of his browlift patients provides a key. His published patient survey report shows that 79.5% of his patients who experienced regular migraines and then undergone a browlift saw dramatic improvement. He continues to move forward optimistically, yet cautiously, noting four main areas in which this surgery might be effective-the septum, forehead, temple, and base of skull.

Today, patients come to Dr. Guyuron not just for cosmetic benefits but for migraine relief. And he is willing to perform browlifts for the purpose of headache relief. He tests first, and only proceeds with surgery only if injecting Botox into the surgery site gives temporary relief.

While Dr. Guyuron has garnered most of the publicity, there are other studies that show similar results, as well. Dr. Ali Sultaneh claims not only that he has had great success with similar surgeries, but that Dr. Guyuron stole his idea. There has been no official accusation (medical advances are often developed in different places in the world simultaneously). No matter who first came up with this discover, such a confirmation of Dr. Guyuron's claims is good news for migraine sufferers.

At the 2001 Congress of Neurological Surgeons, Dr. Sultaneh presented findings on "a simple surgical means of local anesthetization, with no negative side effects, that will cure the ailment [migraines]." And better yet, his study was based on 1,224 patients, far more than the 39 who initially sparked Dr. Guyuron's interest in the phenomenon. Dr. Sultaneh says that he developed his theory around the knowledge that there are certain pressure points that have long been known to provide relief for migraine sufferers.

Possible side effects include temporary hollowing of the temple, numbness, infection or bleeding, phlebitis (that could cause a blood clot to the lungs), localized hair loss in the temples, a whistling sound in the nose and dryness in the nose, but these are all rare.

The surgery costs about $4,000 per site, which means you could be looking at a $12,000 bill. In the long run, however, this might save you money. Migraine medications can easily cost thousands of dollars per year, not to mention the cost of missed work days, and the personal cost to one's emotional and physical well-being.

For now, there are only a few surgeons who perform the technique, but this could change in the future. Now, if only full luscious lips could be a side effect of the surgery that cures insomnia, I would be perfectly happy.

Cant help migraine patients any plastic surgery, but when they remove muscle they remove at the same time branches of extra cranial arteries, that explain why they found that they have a relief from migraine headaches after the plastic surgery that they used to do for plastic reason. While the results will be 100% recovery and without removing any facial or neck muscle, so you can see the truth. In 2000, a Czechoslovakian plastic surgery web sit put in the front page for a years my new migraine discovery as did German web site, and my book was published in 2000, and in 1998 was shown in Syrian TV, and in 1999 also, and it’s a satellite transmition so maybe from one of these he steel my idea or maybe from my abstract in 2001 San Diego. In surgery no one can hide the results, or modify it. Frankly, I am very disappointed that you did not make an attempt to talk to me before you printed his totally unfounded accusation that I am a thief and I stole his idea. Any reports like this would serve the public the best if it is a balanced. You printed his statement without an opportunity for me to respond to his accusation. I am sorry that you are in the middle this unfortunate and again, completely unnecessary feud. I have made a pervious attempt to convince Dr. Sultaneh that he is wrong about his assumption but apparently was not successful. I am making one more such an attempt, but this is the last one.

I was utterly disturbed to receive information indicating that you have continued making disparaging remarks about me. 1- I have never been to Syria or Russia, and I have never met you so I had no way of learning about what you are doing.

2- My first article related to the surgical treatment of migraine headaches was published in July of 2000, whereas your abstract was presented in the USA long after that at a neurosurgery meeting which I do not attend. My first knowledge of your abstract was during the time we did a literature search to write my third article in 2003.

3- According to your abstract, your treatment involved temporal headaches, while my initial discovery centered on frontal migraine headaches.

3- I did not know that you have published a book, nor do I know its topic. I have not seen a Syrian TV show since I was in Syria to give a lecture 18 years ago.

4- If your artery theory is the only reason for migraine headaches and muscles play no role in migraine headaches, then Botox would not be effective in the treatment of migraine headaches.

5- There is absolutely no similarity between what you do and what I do. Surgical attempts to treat migraine headaches is neither your idea nor mine. Dandy in 1931 attempted to treat migraine headaches surgically, with limited temporary success. There have been multiple unsuccessful surgical attempts since. All of my studies were performed with IRB approval and involved one or two unbiased neurologists. The prospective studies included a control arm and were published in peer-reviewed journals. We have followed these patients rigorously. A study nurse coordinator collects the data and I have no role in the data collection or analysis. I hope you have conducted your studies as scientifically as we have.

You do not know me and I feel compelled to let you know that I have written 2 textbooks and over 200 articles and book chapters in peer-reviewed journals, I have developed numerous new techniques, I have served as the president of multiple professional societies, and I am currently the President-Elect of one society and the Vice President of another society. Additionally, I am a director of the American Board of Plastic Surgery. The point is that with all these fortunate achievements, why would I need to steal an idea from you?

As I mentioned in my last e-mail, I could have ignored your e-mail and accusations. However, it seems to me that you have convinced yourself that my surgery is based on your idea, even though there is not an iota of commonality between what I do and what you do. I care about our profession and I do not like any one of us to use insulting words to address each other's actions. Not only do I perform my surgery the way I do, which I have done on over 440 patients with close to 1000 trigger sites, many surgeons across the globe are performing the same procedure without touching the artery and are getting the same results.

We are colleagues, with a supposedly higher level of education and sophistication. You calling me a thief is an incredible insult to me and it is a disgrace to our profession. I am not going to begin a debate with you. However, I urge you, once more, and for the last time, to stop these accusations. If you are still not convinced, I invite you to visit me to observe how I do my procedure and to see that it is in no way similar to what you do, and to see how we conduct our studies with the utmost scientific rigor in order to help you understand that I did not steal your idea.

All plastic surgeons in the world including Dr Bahman Guyuron (http:www.americanmigrainecenter.com), remove frontal muscles for plastic surgery reasons. In 1998, I discovered the simple secret of migraine headaches, by my migraine pressure points: Points # 1, and 2 for frontal migraine. Point # 3 for occipital migraine headaches (BAM). This discovery makes me think to help migraine patients by simple surgery that can be done under a local anesthesia. After my discovery any plastic surgeon can easily observe that migraine patients who were submitted to surgical operation to remove frontal muscles for plastic surgery reasons, that migraine headaches became much better, not because of removing of frontal muscles, but because when plastic surgeons remove the frontal muscles they remove with it branches of superficial temporal artery. Plastic surgeons with these kinds of surgery can't obtain a complete cure for migraine patients because they don’t close the superficial temporal artery in place # 1 before its give 2 branches.
When I close the artery in place # 1 the migraine headaches completely disappears and all accompanied symptoms too, and all frontal muscles remain intact.
